The first three or four days viral sinus infection cannot be differentiated from an early, acute bacterial infection. A pattern of improvement followed by a worsening of symtptoms -- called double sickening-- between days 5 and 10 is more likely to be caused by a bacteria and therefore should respond to antibiotic treatment. Zpak is not the best choice to treat bacterial infections because of increasing resistance.
